If you add them into acad/acadiso.pat, it's simpler having one file, but at least they should all be together at the end or something, to make it easier to copy them out to add into the same file for a newer version. The latter approach makes migration to newer versions easier, but if you have a lot of them, it means a lot of separate files. the file defining the WD4 pattern has to be named WD4.pat. Each pattern needs to be in its own file, and the file name needs to match the pattern name exactly, i.e. Next select the ellipses as shown next to the pattern name to bring up Hatch Pattern Palette with a Custom tab shown.
